[R-G] American deaths in Iraq

I was reading some older posts when I came across these figures --now
certainly out of date -- and I have a couple of questions. Of 776 deaths of
Americans in Iraq, 566 were as a result of hostile action.
1. How did the other 210 die? Friendly fire? Illness?
2. Is that ratio usual in an occupied country --or in war itself?
Not truly important questions, I guess, but I just realized I don't know and
would like to.
