"Two workers died when an explosion caused a roof collapse at a
ConAgra Foods Inc (CAG.N) processing plant in Garner, North Carolina,
and more than 40 people were injured, police said on Wednesday.

"One worker was missing after Tuesday's blast and crews brought in
cranes and other heavy equipment to move pieces of debris to aid in
the search, Garner Police Sergeant Chris Clayton said ....

"The explosion started a fire and blew out a wall that fell onto a row
of parked cars, crushing them. People at the scene reported a strong
smell of ammonia fumes ...."

In view of this tragedy, some elementary questions unsurprisingly are
not being posed.  According to the Occupational Health & Safety
Administration, this plant had nine complaints (i.e., during the
period 10 June 2004 until 10 June 2009) filed against it;  of these,
in only two cases were citations issued (e.g., in August 2006, this
plant was cited for failing to adhere to the "hazard communication"
standard).

What was the exact nature of the violation in August 2006?  Does it
relate to the present tragedy?  Why were the two complaints filed in
2008 closed without enforcement action?  What was the nature of these
two complaints?

Too many questions, not many answers.
